Our Instruction Leaflets inform give patients on how to prepare for the different types of examinations. All the examination information is summarized in these leaflets.
Patients are requested to fill out the patient information and consent form before the examination in some cases. You can print out the information, read it at leisure, fill it out, and bring it along when you come for your examination. Or you may ask our staff to give you the patient information and consent from when you come to DZU, and fill it out here. We will be glad to answer any questions you may have before the examination.
